APPLICATION PROCEDURE
- The application deadline is April 15th of the year prior to the one for which you are applying.
- Complete and submit the application form to the address below. The form may be downloaded from this site: Cytotechnology Application Form, or may be obtained by contacting the Program Director.
- In addition to the application form, the following items must also be submitted:

Official Transcripts
Current official transcript(s) for all college credits must be sent directly to the program (address below). If you have obtained your education outside the United States, you must have your transcript evaluated by one of the approved agencies. You must request the agency send an official DETAILED evaluation report directly to the Program Director at the address below. A list of the agencies and further instructions is available via a link: Acceptable Evaluation Agencies For Transcript Evaluation. All transcripts and/or evaluations must be received prior to the application deadline.

Three Recommendations (Evaluation Forms)
Three recommendations from faculty, professional or business associates should be sent to the program (address below) prior to the application deadline. Recommendations from science-oriented individuals who could address themselves knowledgeably as to your potential for successful completion of this program are preferable. Recommendations should be provided using DMC-University Laboratories Cytotechnology Program Evaluation Form which may be downloaded from this site: Evaluation from Instructor and Employer.

Letter of Interest
A letter of interest indicating how you found out about cytotechnology, what you expect from the program and the profession, etc. must be submitted along with the application.

Application Fee
A non-refundable application fee of $10 in the form of a check or money order must be submitted with the application. Checks should be made payable to DMC-University Laboratories. Please do not submit cash with the application.

- Applicants will be contacted for scheduling of interviews during the months of May-June.
- Upon acceptance into the program, the student obligates himself or herself to remain in training for the twelve (12) consecutive months of the program. The program reserves the right to dismiss a student for failure to meet program standards, misconduct or violation of hospital or program regulations.
